REFERRAL

You will need your GP’s referral outlining your condition for Associate Professor Tsaltas to assess BEFORE we can make an appointment.

Referrals from a GP are valid for 12 months and for 3 months from another specialist.

 

YOUR MEDICAL INFORMATION

Providing all your medical information enables Associate Professor Tsaltas to properly review your medical condition. It also means that any additional information can be obtained i.e. specific testing for your condition.  This way you get the most out of your consultation time.

 

CONSULTING ROOMS

Associate Professor Tsaltas consults in East Melbourne, Mount Waverley & Mornington.

Please call his Main rooms +613 9416 1172 for current locations. Office hours 9.00 am to 5.00 pm main rooms.

Main Rooms East Melbourne Epworth Freemasons Medical Centre Suite 119, 320 Victoria Parade, East Melbourne 3002. Parking is located in the surrounding streets as well as underneath the building.
Mount Waverley Rooms 40 Lemana Crescent, Mount Waverley 3149. Parking on site.	Mornington Rooms The Bays Hospital Suite 8, 262 Main Street, Mornington 3931. Parking on site.

CONSULTATION FEES

Payment is due on the day of your consultation.

We accept VISA/Master Card/EFT and cash. Other fees may be applicable to your consultation depending on what procedures are performed on that day.

First consultation	$400 (30 Min)
Extended First Consultation	$500 (45 Min)
Review Consultation	$220 (15 Min)
Extended Review Consultation	$400 (30 Min)

 

CANCELLATION LIST

If you would like to be placed on this list please let us know and we will contact you when a cancellation occurs.

 

SURGERY

If you require surgery we will provide you with detailed written information including procedure details, consent forms, and a cost estimate prior to your surgery.

We do not participate in Gap cover or Eziclaim health fund schemes.
